Submit a FOIA Request: Pursuant to West Virginia’s Freedom of Information Act, all persons are, unless otherwise expressly provided by law, entitled to full and complete information regarding the affairs of government and the official acts of those who represent them as public officials and employees. All requests for public records containing information relating to the conduct of the public’s business, prepared, owned, and retained by DoHS should be directed to DoHSFOIA@wv.gov.
Submit an Employment Verification Request: To request an employment verification for a mortgage, loan, or general employment confirmation for a current or former employee, please email all requests to OSAOHRMSCAN@wv.gov. Requests must include a signed and dated release form authorizing the verification completed by the employee, as well as a detailed form outlining all required information for the verification. Incomplete requests may result in processing delays.
